Aligning Body with Identity

What is Gender Affirming Top Surgery?

Gender affirming top surgery (also called chest masculinization or FTM/N top surgery) is a surgical procedure that removes breast tissue and reshapes the chest to create a flatter, more masculine or androgynous appearance. It can be performed using a variety of methods, depending on your body and your goals.

Top surgery can be life-affirming—not because it defines who you are, but because it brings your body into closer alignment with how you already feel. Dr. Rose understands that this decision carries weight, and she’s here to walk through it with you at your pace.

What is the Gender gender-affirming top procedure like?

There are several surgical approaches to top surgery. Dr. Rose will recommend a method based on your chest size, skin elasticity, and goals:

  • Double Incision with Free Nipple Grafts (most common for larger chests)
  • Peri-Areolar (Keyhole) (for smaller chests with good skin elasticity)
  • Inverted-T or Buttonhole (in certain cases to preserve nipple sensation)

The procedure is performed under general anesthesia and typically takes 2 to 3 hours. Dr. Rose carefully marks incisions, removes breast tissue, and contours the chest for a natural, symmetrical result. If nipple grafts are needed, they are resized and positioned with precision.

No One Path — Just Your Own

Am I a candidate for gender-affirming top surgery?

You may be a good candidate if you:

  • Are in good general health
  • Have realistic expectations about outcomes and scarring
  • Are emotionally ready for surgery
  • Have been living in your affirmed gender, with or without hormone therapy

Dr. Rose works with patients at various stages of transition and does not require hormone therapy to access surgery. You’ll be asked about your goals, history, and care team to ensure this is the right step for you.

Gender Affirming Recovery & Results

Gender Affirming Recovery

Initial recovery lasts about 2 weeks. Most patients can return to light activity within a few days; however, lifting and upper body movement will be limited initially. Dr. Rose provides a detailed recovery plan including:

  • Scar care
  • Drain management (if applicable)
  • Compression garment use
  • Timeline for resuming work, movement, and exercise

You’ll have several follow-up appointments, and Dr. Rose remains available if any questions arise at any point.

Gender Affirming Results

Final results develop over 3–6 months as swelling resolves and scars fade. Scars can take 12 to 18 months to fully mature. With Dr. Rose’s careful incision planning and scar care, most patients feel proud of their outcome.
